YOUR VISIT
Please Bring Along To Your Appointment: 1. The completed paperwork which may be printed and downloaded below.
2. Two forms of ID, one of which needs to have a photo (drivers
license, state issued ID, passport, work badge, and credit card with photo will suffice). Your insurance card is fine as your secondary ID. 3. Insurance cards
4. Your co-pay (if applicable).
5. List of all current medications and allergies.
6. X-ray films if done at other hospitals or facilities
(you will also be responsible for returning them.)
7. Any medical records or test results pertinent to
your treatment. Please feel free
to call our offices if you need further assistance, or
want to schedule an appointment.
